as soon as you throw the axe you click on the floor to the area you want it to go, I've always understood that bit the only thing I used to have trouble with is that I used to lose all map awareness because I would tunnel vision on catching the axes

They land where you attack move click/or click after you aa. If you stay still it's a 50% chance of the axe landing under you or away from you. And yeah you die more on draven cause he's more of a get in their face champ
